The Physician Assistant (Hospitalist) will provide comprehensive healthcare management of inpatient health conditions, and includes management of required labs, medications, consultation and/or discharge planning. The position is within the Inpatient Hospitalist Service of the Jack C Montgomery VAMC, Eastern Oklahoma VA Health Care System. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Physician Assistant Basic Requirements Master's degree in any discipline or a bachelor's degree in any discipline with one year of experience as a Physician Assistant ~AND~ Successfully passed the PA National Certifying Exam (PANCE) (or are you in the process to taking the exam) ~AND~ Currently hold, or will hold, a full and unrestricted license to practice in a State, Territory, or Commonwealth of the United States or in the District of Columbia. Preferred Experience: Experience with inpatient/Acute care management, bedside procedures (intubation, central line, arterial line, ventilator management) Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met in determining the appropriate grade assignment of candidates. Grade and Step determinations of the selected applicant will be determined by the appropriate Physician Assistant Professional Standards Board and will be based on the selected individuals education and experience. Physician Assistant-I (PA-I) Experience and Education: None beyond the basic requirements. This is the entry level for PA's Demonstrated Knowledge, Skills, and Abilities: 1. Ability to utilize the principles of PA practice in the care of assigned patients to provide appropriate care 2. Ability to perform patient history and physical 3. Ability to formulate a plan and provide treatment to the patient4. Ability to provide patient care that is abreast of changing concepts and advancements in the profession Physician Assistant-II (PA-II) Experience and Education: Two years of successful practice as a PA OR successful completion of a PA post-graduate residency program sponsored by VHA Office of Academic Affiliations or other postgraduate residency programs accredited by VA, ARC-PA, US Armed Forces, or another accrediting agency OR a doctoral degree in a health-related field from an accredited college or university recognized by the US Department of Education. Demonstrated Knowledge, Skills, and Abilities: 1. Skills to analyze and solve complex medical problems 2. Ability to function autonomously and make independent medical decisions 3. Skill to incorporate training and experience into the successful practice of patient care including telehealth medical care 4. Ability to work effectively in an interdisciplinary environment for improvement of services for patient care 5. Ability to practice with colleagues and peers collaboratively Physician Assistant-III (PA-III) Experience and Education: Three years of successful practice as a PA OR successful completion of a PA post graduate residency program sponsored by VHA Office of Academic Affiliations or other postgraduate residency programs accredited by VA, ARC-PA, US Armed Forces, or another accrediting agency AND one year of professional practice as a PA Demonstrated Knowledge, Skills, and Abilities: 1. Ability to clinically evaluate facility patient care delivery systems and validate or identify barriers to safe and effective patient care 2. Skill to consistently demonstrate an elevated level of expertise and autonomy in diagnosing and treating seriously ill, multi-symptomatic patients through facility Focused Professional Practice Evaluation (FPPE) and Ongoing Professional Practice Evaluation (OPPE) 3. Ability to present clinical findings and recommendations to appropriate management officials 4. Ability to clinically facilitate the delivery of patient care in collaboration with health professionals, health managers, and other health care personnel 5. Ability to implement and initiate clinical changes to established treatment protocols based upon new medical literature research findings to enhance patient delivery care for improved patient outcomes 6. The actual grade at which an applicant may be selected for this vacancy is in the range of Grade1 to Grade 3. Physical Requirements: The work requires standing or walking for a major portion of the work day. In the clinics you may need to assist patients into the examinations rooms when in wheel chairs or on carts, which could consist of a variety of standing, walking, twisting, turning, sitting, bending, pushing, carrying and light lifting. This may be considered demanding physical activity. ["HOSPITALIST DUTIES:\nThe incumbent will be part of the Facility's Hospitalist Team and his/her duties include, but are not limited to: Admits and manages inpatients including patients, including in the Intensive Care Unit, with general adult medical illnesses. This includes admitting patients and performing histories and physical examinations; rounding on patients and writing progress notes; discharge planning and timely completion of discharge summaries. Works closely with specialists and primary care Physicians to ensure that inpatients receive continuous high-quality care. Coordinates the transfer of patients with other hospitals if necessary. Provides inpatient internal medicine consultations to other services, including but not limited to, Mental Health, Rehab, and Surgery. Participates in quality assurance programs and chart reviews. Communicates with incoming and off going Hospitalists to ensure continuity of care. Attends Medical Service staff meetings and meetings of the Medical/Dental staff whenever scheduling allows. Serves on Medical Staff committees. Completes all medical records (including discharge summaries) in accordance with Bylaws, Rules and Regulations of the Medical Staff. Ability to participate in the teaching of medical residents and students. Ability to perform minor bedside procedures (Central venous access / arterial line placement/ paracentesis / thoracentesis / intubation/ vent management). Ensure discharge summaries are completed timely. Ideally, the discharge summary will be dictated prior to patient being discharged from facility. Documentation of care in the medical record in keeping with the bylaws of the Medical Center. Provide supervisory oversight residents, fellows, medical students, PA students, as appropriate, who are assigned to the Medicine Inpatient/ER Service. Provide supervisory oversight of the clinical care on the Medicine Inpatient/ER service and ambulatory clinic of Physician Assistants as appropriate. Participates effectively in team meetings and treatment planning conferences and collaborates with multidisciplinary team members in a manner that enhances coordination of comprehensive patient care. Ensures that the practice of medicine is consistent with Joint Commission, CARF, VA policies and procedures, clinical practice guidelines, VISN standards, national statutes and regulations, medical record documentation, mandatory continuing education, and HIPPA policies. Academic/Teaching/Training Involved in performance improvement and quality assurance initiatives. Provides leadership guidance and consultation to staff. Is involved in the supervision and teaching of residents and other trainees assigned to service as appropriate. Serves as a consultant to other medical center staff and trainees and assist in implementation of treatment plan as appropriate regarding their patients. Is responsible for timely completion of all clinical charting and documentation required by the Medical Center or healthcare system, as appropriate. Plans and oversees the completion of personal research programs that might further the educational and mission goals of the VA healthcare system. Other Significant Responsibilities Serve on Medical Center, VISN and/or National VA committees or task forces. Represent the Medicine Inpatient/ER Service on hospital committees as directed by the Chief of Medicine Inpatient/ER, Chief of Staff and Medical Center Director. Maintain CME and re-licensure as dictated by Veterans Administration, State, and the American Board of Internal Medicine rules and regulations.
